Within a week:

You will have an obligation free (NO COST) 30–45 minute introductory meeting with a Support Coordinator.
We will get an understanding of your needs, interpret what your plans mean for you and let you know how we can move forward together.

This is your chance to interview us! It’s about ensuring a perfect fit so that you can achieve great things. It’s your future and your choice, always. This is something we never take for granted.

At the 6 month mark:

Flexibility is key. We know things change, and we work to adapt according to your needs today, to ensure your tomorrow is brighter than ever.

When you engage YES Service, you sign up for an ongoing relationship. We review your progress at every step of the way. This means that if your needs change at any time, we adapt with you.

Whether you need different support or increased support, it is our job to help you put together a review document to submit to the NDIS. This can be done at any time.

9 months and beyond:

We will maintain regular contact with you as you progress; we will help you review your goals leading up to the 12-month NDIS review
(or later if your plan is longer than 12 months). Before the end of your plan, we will meet with you to review your goals and help you gather all the information and reports you may need to ensure you’re prepared and able to put your best foot forward at your next NDIS planning meeting. To do the best, it is important to prepare properly and always think ahead to succeed.

Support Coordinators assist participants in building their skills to understand, implement and use their NDIS plan. A support coordinator helps with planning and engagement of disability support providers.

When you choose YES Service, a Support Coordinator will work with you to understand your goals, requirements and what your current circumstances are.

With our support, we will connect you with the right service providers and community supports.

Our Support Coordinators are professionals with effective experience working with people who have complex life circumstances, helping people to achieve their goals. We work with participants of all ages with different support needs and from all backgrounds.

As Support Coordination experts, our focus isn’t just on translating your plan, it’s about transforming your life.

We offer two levels of support:

Level 2 - Coordination of supports

Our level 2 support helps you to direct your life,
not just your services. We will work together to understand your funding, what to expect from services, and how you want it all designed.

Level 3 - Specialist support coordination

Our level 3 support ensures that if you are a complex situation and have funding for a Specialist Support Coordinator, we can assist you to implement your NDIS plan by finding the right support and addressing any barriers to participation.

We are completely independent. That means we have a broad network of quality providers and will keep researching until we find the right one for your requirements.

We are all about facilitating what’s possible and give you access to a wide range of service providers so you can make an informed choice about who delivers your services.

Our team is passionate about the disability sector. We have industry experience working across health, education, housing, employment, child protection, therapeutic services, social work, in the health sector, residential services, day services, the schooling system etc.

We are NDIS experts, which means we speak the language, understand the legislation and the complex requirements. We streamline, simplify and translate. We have strong relationships with many NDIS service providers and have extensive knowledge of the range of supports available to suit your needs and wants.
